Aktiv Coast Healthcare is committed to protecting your personal information. Our privacy policy is in accordance with the British Columbia Personal Health Information Access and Protection of Privacy Act as well as the federal Personal Information Protection and Electronic Documents Act (PIPEDA).

Aktiv Coast Healthcare Privacy Principles

  1. Consent for collection, use, and disclosure of personal information: The knowledge and consent of the individual, or person authorized to consent on behalf of the individual, are required for the collection, use or disclosure of personal health information, except where otherwise required by law. Exceptions include the potential for harm to oneself or others.
  2. Limit the collection of personal health information: Aktiv Coast Healthcare will limit the collection of personal health information only to what is necessary for the services provided. Any personal information collected online will only be used for the purpose for which it is specified.
  3. Individual Access to Personal Health Information: as per the Personal Health Information Privacy Act patients have the right to request, examine or receive a copy of his or her personal health information in electronic or physical format.
  4. Identifying purposes for collecting personal health information: Aktiv Coast Healthcare will identify the purposes for which personal information is collected at or before the time the information is collected.
  5. Accountability: Aktiv Coast Healthcare is a health care custodian and has a designated privacy officer (Dr. Tyler Linn) accountable to ensure that our privacy policy is upheld in accordance with the British Columbia Personal Health Information Access and Protection of Privacy Act as well as the federal Personal Information Protection and Electronic Documents Act (PIPEDA).
  6. Ensuring safeguards for personal information: Personal information in paper format is only stored under direct supervision or locked in a secured and restricted area until it is uploaded to a secure server. All practice records are stored on Canadian servers to ensure accordance with Canada’s privacy legislation.
  7. Accuracy of personal information: Personal information will be as accurate, complete and up-to-date as is necessary for the purpose for which it is intended.
  8. Limiting use, disclosure, and retention of personal information: Personal information will not be used or disclosed for any reason other then its intended purpose. Personal information will be retained only for as long as necessary for the fulfillment of those purposes and to meet the regulatory guidelines necessary for health professionals in the province of British Columbia.

All paper information is shredded and disposed of appropriately after being securely uploaded to our server.
